How to Receive Community Service Hours:

If you are not from Milpitas High School:
1. We will fill out Section 1 for you, but it is your responsibility to fill out the rest of the form.
2. For section 3, you may list the email of the program (growth mindset.info@gmail.com) and parent of the tutee.
1. You must explain how community service in your school works when you complete the tutor application form.
2. If it is difficult to explain or you have too many links to send, then email or DM us.
For All Tutors:

1. Before receiving your community service hours, you must officially resign from the program by completing the Resignation Form. This is to organize community service hours for everyone.
2. You must resign by the first day of each month. Not completing the resignation form indicates that you will continue to tutor for the month.
For example, if you do not complete the resignation form by the 1st of October, then you have indicated that you will continue to tutor your tutee for the entire month of October.
3. If you complete the resignation form by the first day of the month, please give our team about a week to send your community service form with your completed hours.
4. If you would just like to take a month or so off from the program, you can always rejoin by completing the tutor reapplication form.
REMINDERS:
1. Record your sessions so that you may be able to provide additional proof that you have completed lessons.
2. Complete the Log Hours Form within 24 hours of your tutoring session.
3. Also make sure you know the contact information of your tutee to verify community service with your school.
